Community and Employment Partner’s services are designed to support adults with disabilities to live and work as independently as possible in their communities.
Community and Employment Partners supports individuals that are interested in living independently to gain the life skills essential for finding and maintaining a job that fits their individual needs and skills, maintaining a home, a budget, health and happiness.
Our trained Case Workers and Case Managers work one-on-one with individuals in both the community in which they live and work.
Our staff serves as a liaison between the individual and their identified needs and preferences. By assessing each person’s individual needs, supports and resources are tailored to assist in the transition towards established life goals such as a new home, a new job, or new life experiences.
We can support you with learning how to locate and use resources in your community to achieve what is important to you. Step by step, staff take time to understand your abilities and build skills to realize your life goals.
All services are provided in the community where you live and work.
