An Overview of Community and Employment Partners

Community and Employment Partner’s services are designed to support adults with disabilities to live and work as independently as possible in their communities.

 

What does Community and Employment Partners offer?

Community and Employment Partners supports individuals that are interested in living independently to gain the life skills essential for finding and maintaining a job that fits their individual needs and skills, maintaining a home,  a budget, health and happiness.

 

Where are these services offered?

Our trained Case Workers and Case Managers work one-on-one with individuals in both the community in which they live and work.

 

What role do the Case Worker’s and Case Manager’s take in working with the individuals in the program?

Our staff serves as a liaison between the individual and their identified needs and preferences. By assessing each person’s individual needs, supports and resources are tailored to assist in the transition towards established life goals such as a new home, a new job, or new life experiences.

 

What can Community and Employment Partners do for me?

We can support you with learning how to locate and use resources in your community to achieve what is important to you. Step by step, staff take time to understand your abilities and build skills to realize your life goals.

 

Where will my services be provided?

All services are provided in the community where you live and work.

 

Can Employment Partners help me find Employment?

Yes, we will work individually with you to locate employment in the community according to your preferences.

 

What goals can Supported Employment help me with?

  • Assessing jobs of interest

  • Completing and submitting employment applications

  • Creating a resume

  • Preparing for interviews

  • Following up with employers

  • Job coaching

  • Job advancement

What goals can Community Partners help me with?

All of the goals will be based on your preferences and desires. Services may include:

  • Budgeting and banking

  • Planning and preparing meals

  • Obtaining health/ medical care

  • Locating housing

  • Seeking recreational social activities

  • Transportation training

  • Locating and coordinating community services/ resources
