Adult Life Planning
What is Adult Life Planning?
Adult Life Planning will assist young adults who will be leaving the Autism Waiver by providing specific interventions and supports for the transition from school based services to the adult services delivery system.
Our Adult Life Planner will provide information to young adults and their families on the eligibility criteria for Federal, State, and other community services and supports. This service provides parents with direct, hands on technical assistance and support to improve their understanding of services and supports available for adults with autism by focusing on the shift from a child-centered developmental model to an adult oriented model of self-determination.
We will provide the technical assistance and support for young adults and families to develop a plan for self-determination, independence, choice, community integration, Circles of Support, and decision making in the adult developmental disabilities system. We work with each participant in their home environment to identify skills related to self-advocacy, self-direction, natural supports, and the adult service system’s employment options.
Who will be the Adult Life Planner?
Cheryl A. Johnson, a Licensed Certified Social Worker with over 25 years experience in the field of developmental disabilities including Developmental Disabilities Administration (DDA) funding, will function as the Adult Life Planner with guidance from the Director of Autism Services, Tiffany Noël Hiob, LCPC.
How do I receive Adult Life Planning?
If your child is between the ages of 18 and 21 and you are interested in additional information about Adult Life Planning then please notify your Waiver Coordinator and contact Abilities Network Autism Services to schedule a brief meeting to discuss eligibility and the needs of your son/daughter.
